The Global Community Resource Centre is a dynamic ‘one-stop’ interactive hub for patients, families and the community to access reliable, current multilingual health and community information in a variety of formats, including print, online and through interactive workshops. Our goal is to empower visitors to the Centre with increased health literacy and better access to community services.
The collaborative model for the Centre includes partnerships with eight community agencies who staff a ‘Community Desk’ on a rotating basis and provide patients and visitors with information on community services in Scarborough.

These eight community agencies have partnered with SHN on the Global Community Resource Centre. They staff the ‘Community Desk’ at the Centre on a rotating basis that provides clients with information on health and community services in Scarborough. The agencies, along with SHN staff, also provide multilingual workshops on a range of topics of interest.
Agincourt Community Services Association is a non-profit, multi-service agency located in the heart of Agincourt, which addresses needs and empowers children, youth, newcomers, homeless and underserved communities to build a better tomorrow.
CCS is a non-profit agency providing services that assist in the settlement and integration of immigrants and refugees.
Your Local Health Integration Network provides a range of services through Home and Community Care (HCC), including:
- In-home services (nursing, physiotherapy, occupational therapy, speech-language therapy, social work, dietetics, medical supplies and equipment, and personal supports such as bathing and dressing)
- Specialized programs for children, mental health and addictions for youth, palliative care, and Tele-homecare
- Referrals to community resources, such as adult day programs, meal delivery services, and transportation assistance
- Assessment for long-term care, convalescent care, respite services, Assisted Living for High Risk Seniors

Settlement Assistance and Family Support Services (SAFSS) is a non-profit, charitable community based organization providing multilingual and multicultural services to the community at large.
Senior Persons Living Connected has been a pioneer in the field of senior care for over 30 years. St. Paul’s unique blend of housing and community services provide a continuum of care that supports independent living for those in need of extra assistance and care. The programs and services help to attain a higher quality of life, maintain seniors’ independence and overall well-being, and facilitate seniors’ navigation through a healthy aging process.
Toronto Public Health (TPH) works in many ways to improve the overall health of the population and to overcome health inequalities. They provide services to individuals and communities, and advocates for public policies that makes Toronto healthier. TPH services are funded by City Council and the Ministry of Health and Long-Term Care. They are governed by the Board of Health, comprised of elected councillors and citizen representatives. They strive to make services accessible and equitable to all residents of Toronto.
TransCare Community Support Services provides home and community support services for people who need assistance due to illness, convalescence, disability or functional limitations related to aging.
