LINK Program

General information

The LINK Program at SHN supports young adults between 18 and 24 years old who are experiencing difficulties related to significant mental illness. We work directly with community partners to assist clients to connect with the services and resources they need to transition to ongoing supports and services in the community.

Our LINK team includes two clinical navigators that work within an interdisciplinary team to assist youth to bridge the gap between Child and Adolescent Mental Health Services and Adult Mental Health Services.

Clients will:

  • Be 18 to 24 years old
  • Be able to navigate to appropriate services within 6 months to 1 year
  • Have significant mental illness/impairment requiring stabilization
  • Fall into service gaps, such as not having current psychosocial supports and not being connected to a psychiatrist

This is a short-term treatment program, typically between 6 months and 1 year.

Success in the LINK Program means:

  • The treatment goals are met
  • The individual is connected to appropriate adult supports and resources
  • Individuals are aware of what resources are available within local community
  • Individuals are able to participate more fully in their activities of daily living

Our services

The focus of the LINK program is to support patient recovery through partnerships, navigation, and supportive interventions.

What we offer

Individual interventions

Individual interventions are developed with the client, assessed and revised regularly during check-in appointments.

Group interventions

The LINK Program offers a drop-in and a psychoeducation group therapy once a week.

Service navigation

Our team will offer recommendations and referrals to alternative resources as needed.

One-on-one appointments with clinical navigators who connect clients with the appropriate resources within the hospital system or community for their treatment.

Psychiatric consultation

Psychiatric consultation is provided as needed.

Getting started

A family doctor or paediatrician can refer a client to our program through Ocean eReferral or by faxing us our Child Adolescent Outpatient Referral form (see below).
