Heart Function Clinic

The Heart Function Clinic at Scarborough Health Network (SHN) manages patients’ heart function care on an outpatient basis to help reduce emergency department visits and hospital admissions.

The interprofessional team, which includes a cardiologist and nurses specializing in cardiac care, works together to manage heart health status through physical assessment and ongoing teachings about medication and diet, as well as through the monitoring of symptoms and conditions.

What We Offer

The heart function team provides services, including:

  • Instructional materials and demonstrations to help manage and monitor heart health
  • Ongoing telephone support for patients and caregivers
  • Collaboration with other members of a patient’s health care team

Accessing Care

The Heart Function Clinic is open for appointments on Monday to Thursday from 8 a.m. to noon each week. To refer a patient, please fax to 647-598-4021. For questions or to learn more, please call 416-438-2911 ext. 6016 between 8 a.m. and noon. 


The Heart Function Clinic serves adults with a known or suspected diagnosis of congestive heart failure. Please ask your family doctor or cardiologist to refer you to clinic.

What to expect at the Clinic

During your first consultation, your care team will conduct a physical examination and review your medical history. Our consultations are performed by cardiologists and nurses who provide specialized cardiac care.

We ask that you bring a list of the medications, vitamins and herbal supplements that you take, along with information on dosages. We use this information at the time of your first visit to get a complete picture of you and your overall condition in order to develop an appropriate treatment plan.
