GENERAL INFORMATION

Scarborough Health Network’s (SHN) Nurse-Led Outreach Team (NLOT) is a collaboration with Ontario Health and Long-Term Care (LTC) homes to support care for elderly patients in our Scarborough community.

The NLOT team consists of Nurse Practitioners and Registered Nurses who work closely with LTC staff, patients and families to support transitions from hospital and to restore health through assessments and treatment for those with complex needs within LTC homes. Additionally, they provide capacity-building, reduce transfers from LTC to the Emergency Department and augment palliative care supports for those who can be treated safely within LTC homes.

Our services

Hospital Transitions: Hospital in-patient assessments and consultations for LTC residents to ensure a smooth transition to Long-Term Care Homes.

On-site resident support: Resident assessments and interventional support in the Long-Term Care Home by the NLOT team within 24-48 hours. Virtual care consultations are also available.

Education for healthcare providers: Skills development and training for healthcare providers working in Long-Term Care Homes, including assessments and interventions.

Connections to additional SHN services: We facilitate connections to specialty services, both internal and external to SHN.

What residents and families can expect from NLOT

Our NLOT nurses will work closely with LTC staff to provide compassionate and personalized care when additional support is required by the patient’s LTC, providing they are a resident of an LTC that SHN has partnered with.

The team is committed to building relationships of trust with residents, ensuring that their individual needs and preferences are met with respect and dignity. They engage with residents through open communication, and collaborative care planning, involving both residents and their families in every step of the process. Their goal is to work with the resident’s LTC to create a supportive and inclusive environment where residents feel valued and families feel reassured about the quality of care their loved ones receive.
