Cancer Care Ontario has notified hospitals across Ontario that there may have been unintended error in the dosage of specific chemotherapy drugs given to certain patients. The way a small group of intravenous (IV) chemotherapy drugs were given could have led to some patients receiving a lower dose than what was ordered by their doctor. This happened because some of the medication may have been left in the IV tubing without actually getting into the patient’s body.

Delivering safe, quality care is our highest priority. As soon as we became aware of this issue, we promptly investigated our practices and patient files, and have taken all steps required to ensure patients are now receiving the correct dose of their chemotherapy medication.

If you are currently receiving treatment at either the Centenary or General site and were identified in this review as requiring follow-up, you have already been notified. If you or your family member received treatment at either the Centenary or General site in the past, our cancer specialists have reviewed the treatment you received and have determined that your treatment was very unlikely to have been impacted.
