We are proud to announce the launch of the Scarborough Health Network (SHN) Cancer Care Journey Toolkit – a new, patient-centered resource designed to empower individuals navigating their cancer care journey across our network.

This Toolkit is more than just a guide – it’s a step forward in our commitment to health equity. Developed in response to the 2023 recommendations from Ontario Health – Cancer Care Ontario (OH-CCO), the Toolkit directly addresses the systemic barriers and service gaps experienced by equity-deserving populations. It reflects our shared responsibility to ensure that every patient, regardless of background, has access to clear, inclusive, and supportive information throughout their care.

What’s inside the Toolkit?

These tools are designed to walk patients through each stage of their cancer care experience, helping them feel informed, supported, and in control.

Why this matters

The Toolkit was born from a collaborative effort led by our interprofessional team – including the Indigenous Patient Navigator and Oncology Social Worker – who worked closely with patients and families to understand their needs. Their insights shaped a resource that is not only comprehensive but also culturally responsive and accessible.

This initiative was made possible through funding from OH-CCO and the Canadian Partnership Against Cancer (CPAC), secured through SHN’s successful Expression of Interest in 2023. The three-year funding supports our broader efforts to embed equity into every aspect of cancer care.
